This role includes triaging hospice calls, managing the referral and intake process, and providing field visits as needed. The ideal candidate is calm under pressure, confident in decision-making, and comfortable working independently while collaborating with a broader interdisciplinary team.Key Responsibilities:Triaging after-hours and weekend hospice calls to ensure timely and appropriate interventionsSupporting the intake and referral process with compassion and efficiencyCompleting in-person field visits when necessary to assess patient needsAccurately documenting in the EMR system and communicating with the care teamProviding holistic, patient-centered care with sensitivity to emotional, physical, and spiritual needsQualifications:Current LPN license in PAHospice experience requiredExperience with EMR systems requiredProficient with computers and comfortable navigating digital toolsStrong interpersonal and communication skillsAbility to work independently and prioritize tasks effectivelyReliable transportation, valid drivers license, & auto insurance for travel within the service areaAvailability to work Saturday and Sunday 8a to 8pby Jobble
